Remote Tutoring Registration Form – Children

Remote tutoring services are available Monday thru Friday, after registration is completed and by appointment only.

Once the registration form is submitted, we will contact you by telephone to schedule your remote tutoring session(s).

Your appointment is guaranteed only after you receive confirmation from ILC of full tuition payment.

CANCELLATION POLICY: Remote tutoring sessions must be cancelled at least 2 hours prior to scheduled session. If proper notice is not given in a timely manner you will be charged for the session.

By signing this form, you are agreeing to the ILC payment terms and conditions to pay your child’s tuition as indicated above. If tuition is not paid in a timely manner, the ILC can consider the registration void.

You are also stating that all information provided in this form is current and correct. Must be 18 years of age or older to register.
